Regular (Feb. 1 - Apr. 22, 2005):	$375
Late and on-site:	$400

Cancellation Policy
Request for refunds may be made by submitting a written cancellation letter to
The Task Force for Child Survival and Development by March 28, 2005. The
cancellation letter may be submitted via fax to 404-371-1087 or by mail to the
address below. All other refunds must be approved by CDC Division of
Laboratory Services. Substitutions will be allowed as appropriate.
