Registration Fees and Deadlines
Early (until January 28, 2005): $150
Regular (until March 4, 2005): $175
Late and on-site:$200
One-day:$100

Early registration has ended. All new registrants may only register as regular
or one-day.-January 30, 2005

Cancellation Policy
Request for refunds may be made by submitting a written cancellation letter to
The Task Force for Child Survival and Development by February 21, 2005. The
cancellation letter may be submitted via fax to 404-371-1087 or by mail to the
address below. All other refunds must be approved by the National Immunization
Program. Substitutions will be allowed as appropriate.
