Regular (until February 17, 2006):	$200
Late/On-site (after February 17):	$225

Cancellation Policy
Request for refunds may be made by submitting a written cancellation letter to
The Task Force for Child Survival and Development by January 30, 2006. The
cancellation letter may be submitted via fax to 404-371-1087 or by mail to the
address below. All other refunds must be approved by the National Immunization
Program. Substitutions will be allowed as appropriate.
